Special Terms: To qualify for the DHCC Special Implant Program you must agree to complete the restoration of the implant(s) at our DHCC facility. This may include a single appliance and crown or involve the restoration of several teeth and implants with a denture or bridge. The necessary treatment will be determined after a complete oral exam, x-rays and diagnostic models. Some patients may require a medical exam from their physician if a health condition could compromise their dental care.

Additional Procedures: Some patients may require additional procedures to ensure the success of the implants, especially if there is inadequate bone in the area planned to receive the implants. These procedures include reshaping the gum and bone, removal of diseased teeth and tissue, and placement of grafting material. These procedures will usually incur additional fees. If you require any of these additional procedures they will be included in your treatment plan.

Optional Services: you may elect to have the operation performed with sedation or an intravenous anesthetic. Under most circumstances sedation or general anesthesia is optional, although some procedures are invasive enough that the doctors may require some form of sedation or general anesthesia.

Detailed Treatment Plan: A detailed treatment plan will be presented to you prior to the beginning of treatment and we will endeavor to include all anticipated procedures and expenses related to the completion of the implant(s) restoration. Routine post-operative care is included at no extra charge. Unusual or extenuating circumstances or complications may require an additional fee. The patient is responsible for medications, x-rays, and other ancillary treatments not specifically included in the treatment plan.

"No Surprises" Policy: You will be informed in advance of any procedures that you will be billed for. We will not preform a procedure and then inform you afterward that you have incurred a additional charge.

Payment: Financing options are available with approved credit and payment with "CareCredit" or credit cards is accepted (an additional finance handling fee may apply). Options for payment will be discussed prior to making the next appointment.

Incomplete Treatment: Should you decide not to complete your treatment plan at Dental HealthCare Centers, the implants that have been placed will be repriced at our usual fee of $1,950 per implant and the balance of the account refunded to you.

First Visit: Our fee for an implant consultation is $108.40. Additional fees for x-rays ($98.25 and up) and diagnostic casts ($96.50) are required if current diagnostic x-rays or models are not available.
